Collection of Information from Students Affected by the Recent Floods

Dear Students,

The Disaster Management Committee of the University of Jaffna (UoJ) is gathering information on students and their family members who have been affected by the recent floods. This information will help us understand your circumstances and coordinate support where possible.

Please provide accurate details so that we may contact you if needed. All information will be treated with strict confidentiality and used solely for support and reporting purposes.

Students who have been affected are requested to complete and submit the form before 1.00 p.m. on 5 December 2025, to facilitate the timely compilation of information.

Attention:

  • If you are not affected, you do not need to fill out this form.

  • However, if you are aware of another student who may have been affected, you may complete the form on their behalf and provide your mobile number for follow-up.
